Born from a need to offer healthcare services to men struggling with insufficient housing and/or in recovery for substance use disorder, the Changed Lives Mobile Clinic is a nurse-managed interprofessional collaborative clinic for medically underserved patients in central Alabama.
The UAB School of Nursing partners with The Foundry Ministries to offer ambulatory care services in two of the Foundry's five treatment facilities. Changed Lives Mobile Clinic is a free safety-net clinic that is funded through charitable donations. All services, including medications and point-of-care lab testing, are offered at no cost to the patients.
Mission: To provide ambulatory care services and health education to medically underserved patients.
Interprofessional Team: Nurse Practitioners, Registered Nurses, Collaborating Physicians, Social Worker, Case Managers, Pastoral Care Counselors.
Person-Centered Care Model: The Changed Live Mobile Clinic provides convenient healthcare access by offering patients healthcare services in their place of residence.
Referral: Patients must be a program participant in The Foundry Ministries Rescue and Recovery Programs. The Foundry Ministries reaches thousands of people through our Rescue and Recovery Programs. Each year, 600+ individuals come from all over the Southeast for substance use disorder recovery. The ministry also meets the physical needs of homeless and low-income families through the Rescue Program.

Clinic Location: The clinic is a hybrid model offering telehealth services weekly and in-person care monthly. The care team travels to the residential treatment facilities in North Birmingham and Cullman Alabama to offer care services.

The University of Alabama at Birmingham School of Nursing and the Cooper Green Mercy Health Services Authority partnered to expand upon the women’s health clinic, offering more advanced services to underserved patients across Jefferson County. Opening in February 2023, the clinic offers a variety of women’s health services including primary and preventive care, as well as diagnosis and management of various gynecological conditions for reproductive age, perimenopausal, and post-menopausal patients. The clinic also provides counseling for all sexual and reproductive health needs.
The clinic is open on Thursdays at Cooper Green and is run by Kelley Borella, DNP, CRNP, WHNP-BC.
Under the direction of Michele Talley, PhD, ACNP-BC, FAANP, FNAP, FAAN, the University of Alabama at Birmingham School of Nursing partners with UAB Medicine to operate the nurse-managed PATH (Providing Access to Healthcare) Clinic at UAB.
This collaborative effort utilizes an interprofessional, team-based model to provide diabetes care at no cost to an uninsured, medically underserved population discharged from UAB Hospital.
Interprofessional Services include visits with Nurse Practitioners, Physicians, Optometrists, Social Workers, Behavioral Health Specialists, Registered Nurses, Dietitians, and Physical Therapists. Assistance in obtaining needed supplies and medications at the lowest possible cost is also provided. Students from a variety of disciplines are integrated in the clinic to learn in an innovative interprofessional model of care.

Diabetes patients being discharged from UAB Hospital: Referrals of patients who are uninsured and have an A1c > 9 can be made using an AdHoc form in the IMPACT system. The care coordinator team will contact the patients who are referred to schedule their first appointment within 2-3 weeks. Born from an academic-practice partnership, the Heart FailuRe Transitional Care Services for Adults (HRTSA) Clinic is a nurse-managed interprofessional collaborative clinic for underserved patients with a diagnosis of heart failure.
Mission: To provide guideline-directed care and education to underserved patients with heart failure and their caregivers in an interprofessional collaborative practice across care transitions.
Interprofessional Team: Nurse Practitioners, Clinical Nurse Leader, Social Workers, Registered Nurse, Certified Medical Assistant, Collaborating Physician, Behavioral Health Services, Pharmacist, Health Professions and Public Health
Transitional Care Model: The HRTSA Clinic provides services across the care continuum by seeing patients in the hospital, home, and clinic.
Referral: Patients must have a diagnosis of heart failure and be underserved.
