Environmental Health & Safety has launched the new EHS Assistant (EHSA) system. To ensure ease of adoption and understanding, it is rolling out in phases. During each phase, EH&S will contact and arrange an orientation followed by access to the system. Access to the system will not be granted until orientation is complete. For questions, contact EH&S at EHSASupport@uab.edu

  1. Login to EHSA: https://ehsa.fab.uab.edu/ehsa
  2. Click EHS Assessment (bottom right corner) on the EHSA home page.

  3. Click Browse New Forms (top left corner)

  4. Click Start next to Controlled Substances Use Authorization Request

  5. Click on the drop-down menu next to
  • Researcher> and select the PI name
  • Permit > and select the CSUA Permit to renew.
  • Under Amendment Type: click on

    Click the green Create Assessment button.

  1. In the Starting New Assessment window, you will be asked if you would like to copy your previous responses into this assessment.
    Click Yes.

  2. Under the Assessment Questions tab at the top, please review and update all information under the following tabs:
    • Personnel Information
    • Protocol Information
    • Storage Location

 

  1. Click Sign to add your information to the signature line.

  1. Click Save as Complete to submit the form >
    click Yes when asked if you would like to proceed.
  2. Click OK in the resulting notification window.
