Recertification of Effort

This form is to be used when requesting a recertification of a previously certified effort report or correcting an uncertified effort report after the period close date.

It is important to provide complete and correct information; partial or incomplete responses will be returned to the departmental contact listed on the form. Once approved, this document will decertify or cancel the previous effort report and serve as the certified effort report.

This form, the information provided, and referenced support documentation may be subject to review by your School, Financial Affairs, the University Compliance Office (UCO), UA System Office of Internal Audit, and external auditors. In addition, this may be subject to audit by federal and state agencies.
